Steps:
It's basically a series of seven tasks that you run for him, mostly kill tasks. Having a Skald with you would cut the time to do it in half.Talk to the start NPC:
Hib: Stiabhan (weapon merchant) on the Stygian Delta
Mid: Rejmar (an alch merchant) in Stygia Haven
Alb: Trevor in Stygia
Scorpion- Travel west to Land of Atum, loc 30k,54k
Black Croc- Travel west again along coast line this time to Land of
Atum, loc 64k,37k.
Icaculia- West of Haven, snake like creatues.
Once you kill the two iacus (snake right outside the city going west), the starting NPC gives you the task of finding his lost brother. He says to search east of town on a small island.
East, SE of town there is an island looking like a solid stone and behind it is another smaller island looking all sandy and that is the island you want. Please beware of the red/purple crock people and the river aggro all along the way.
The loc of the island is 56763, 62240. Once ashore do /search and find a note.
Return to town and hand your NPC the note, and he will tell you to find Ruseth. After he gives you that assignment, you should right-click on the start npc again and /s Ruseth. When you do this, you create Ruseth to spawn.
Go to 12384, 49261, Stygia Delta by the Mau village and he is standing there. Talk to him and run back to town.
setian- You can kill any setian to get the ears. We found our setian at Stygia Delta 9.4k, 30.6k. They can also be found at 45k,28k Land of Atum. Is only a night spawn at this location, or at least only spawned for me until night set in. Setian can also be found during the day in at loc 13.4, 32.0 in Stygian Delta.
The kill tasks are yellow to orange cons at 50, and running these tasks is actually not too bad since it lets you explore quite a bit around the Stygian Delta and the Land of Atum. The reward is the following outstanding freaking bracer plus 1.5 BILLION XP (at level 50). Another great quest to do at lv 49 or so now probably.

Tips:
The group I was in did it with a NS, Menty, Druid, and Bardbot. There is a bit of running around.. so speed was nice =). You have to cross the river on some of the steps, some aggro down below the surface of the water.Drops:
